Assumption Parish 911 has upgraded the 911 System. In the event you have an emergency we would like to have as much information as possible about the residents at your address. Please fill out the Premise Information Form to register the residents at your location with 911. We are encouraging everyone to fill out this Premise Information Form. You can download this form and fill it out. The form can be submitted to us by hitting the send button or you can mail it to the following address:
